The medical form provides SOAR NW with specific medical histories and issues for each of the guests. Allergies and medications are imperative for us to know. This information will be due upon final payment. (Significant food allergies and dietary needs should be discussed, and we are typically able to accommodate these w/ enough time to prepare)
